this is the vent. It appears to be a 1/4 mounted on a 1/2 adapter.
So this is on the boiler side, I just need to shut off the isolation valves on the boiler side, drain the water a few drops to relieve the pressure and then unscrew and replace right?

What make sense and what they do is two different things. Gauges cost money x the # of units they sell makes it a no-go. Everything they do, cheap overseas capacitors, microchannel coils and 1 pole contactors that should be 2 pole and no pressure switches that could save a compressor on refrigerant loss……they won't put gauges on. They have a planned life cycle.

Even if you intend to put 1/2" PEX through 3/4" PVC you may be severely disappointed. 1/2" PEX has an outer diameter of about 5/8". Between the bend radius and any excessive glue that oozed into the conduit you may never get it through.
With electrical conduit work they only allow 40% (3 or more conductors), 53% (1 conductor) fill of the conduits available cross-sectional area.
As @mattmia2 stated you need to think much bigger. And use electrical conduit type radius not water pipe type radius.

how did you test the chloride level, what is that number? What is the HTP limit?
Chlorine level tested with color strips?

Is this a HERS rated house? I've never heard of using an HRV as bath exhaust. They're either ducted to the living space, or ducted to the HVAC system. The normal practice is, if no HRV, then bath exhausts on timers to get the air exchanges. And this is one HRV serving three bathrooms, or each bathroom has its own HRV?

If the system is allowed to sit idle for a while, do the temperatures the system displays for Sensor 1 and Sensor 2 seem plausible (basically approaching ambient temps) ? If so I still think it's a water flow issue through the heat exchanger.

When you say "sleeve", you mean running the tubing through the top of the double tap bushing? With a compression adapter to lock it in place? Is there a 1/2" rigid pipe threaded to the underside of the bushing so the tubing is forced straight down, rather than curling?
That rigid pipe (L copper or galvanized) can be used as the pick-up tube itself. No need to sleeve through. Just put a flare adapter on the top side, and its a sealed connection. I did my Roth that way.

As you sure there isn't any water in the tube as you try to heat it. Even a trickle, fast drip will prevent the tube from getting hot all around.
Jam a few slice of Wonder 🍞 Bread up the tube to temporarily stop a small flow. That's why they call it Wonder bread.
