If you choose not to insulate above the subfloor then you should be fine.
I assume you plan on using 5/16” PEX tubing (laid into the 1/2” Quik Trak panels), is this correct?
To your other questions:
If you choose to use Quik Trak panels, I would consult with the tiling contractor you choose, and allow him\her to work with a system that they are very proficient at installing.
If it were my house (and I was dead-set on radiant tubing), I would install the tubing into Joist Trak panels beneath the subfloor, with 3/4” AdvanTech as the subfloor. Install Schluter decoupling membrane, thinset, then tile. This typically equates to a finished floor height of 1-1/2” (which is almost perfectly level with a typical built up floor height using 3/4” ply and 3/4” hardwood).

Did the radiators you are diagnosing ever work correctly ?
IMO there are many things that are not correct or at best very odd with your system.
One example; The one of possibly two main vents (two mains at the boiler) should not be puking and staining the wall.
The steam should leave the boiler through the larger insulated pipes and go to each radiator. The way it looks the radiator's condensate should return to the boiler through the un-insulated copper pipes.
There should be a means of limiting or restricting the steam from getting into the returns. Just an elbow at the condensate end of the radiator may not work so good. Since if steam gets into the returns it may trap air in some parts of the system and the part with the trapped air will not heat properly.

it’s possible that the system vented into the chimney and that was used to ensure it condensed before it reached the chimney, assuming it was a 2-pipe steam system.

Only problem with a loop seal is if someone jacks the pressure up. Other than that, more reliable than a trap.
