And to add to that, in order to find the manufacturing date on the circ look at the electrical box near the wire knockout. It will have a date such as 0506 which would mean it was probably manufactured in Jan or Feb of 2006 (yes the circs get predated at manufacturing so that when you get them in your hands its close if not early for the jobsite)

was the pre-charge checked and adjusted on both tanks?
What was the capacity of the old compression tank?
Did water come out of the air stem on. The tank that failed? That is an indication of a failed diaphragm
Has the pump been changed? Looks like you are pumping at the expansion tank? Pump on the return, expansion connection on the supply? Where is the fill valve connection?

If you're replacing a boiler where there's a mono flo system, its important supply and return flow stay the same. Where in a series loop it doesn't matter if you reverse flow in most cases. I've replaced numerous boilers where's there's a mono flo system and the old had the B&G 100 on the return. New installs always pumping away. Never an issue.

And the behaviur of a monoflow system has nothing to do with the location of the pump nor even the static pressure in the system. It does, however, have a lot to do with the water velocity in the distribution pipes — so one way to mess it up which your guys may have found is to substitute a very different pump…

just to be clear the pump could be on the return and still be pumping away as long as the expansion tank is connected at or near the pumps inlet
Many mod cons have the pump on the return to help overcome pressure drop in the boiler
Here are two examples, one with pump at return, one at supply
Typical ci boilers have low pressure drop so tank at return, pump on supply is still “pumping away”
What is important is for the air purger to be at the boiler supply out, the hottest point in the system

Rooms with a lot of glass tend to feel "cold" even though the ambient is warm. Your body heat is being pulled to the cold glass. AKA the "cold 70 effect"
Use an accurate thermometer to adjust the wall thermostat reading. There is a differential in the thermostat 1.5- 2° that may be adjustable also. Pull out the manual on your thermostst.

On your mains I would suggest at least 2, 3 and 4 Gorton 2's respectively on the A, B and 2nd floor mains.
