Does the cost of replacement increase to twice the cost of the boiler alone, with the near boiler piping and labor to do so?

Frequently more than 3X when all the piping and controls are considered. The boiler itself is not all that costly.
It's the labor and all the piping and controls that really add to the final cost. Could be 4X if the job is done really well with a drop header and generous sized piping (NOT 2"!!)
It takes 40-48 man hours to properly remove and install a new steam boiler with all associated piping and controls and startup and some skim. If it is not planned perfectly, with every possible needed pipe and control, the number of man hours climbs from there because it becomes multiple trips to the supply house(s)
The price per hour is depending on where you are located. Make your own estimate and calculate the total labor.

Much more than twice the retail price. Domestic iron piping is quite pricey and slow going.
And Labor rates depend mostly on the cost of overhead where you are. There you get what you pay for.
And if you can afford it, you should look outside the boiler room and correct any problems that hacks and time have invested into the system: Old mismatched painted radiator vents, leaking valves and ancient rusty wet returns. It may be the first time anyone's putting work into the system in eighty years.

Old rule of thumb from way back was to take the price of the boiler and double it.
That is not enough. Taking the price of the boiler and all the material needed to install it and doubling that may get you close.
Or the boiler cost x 3
Either way it doesn't matter. Labor prices vary from area to area.
Some installers do a great job and some do the old cut and slide.
You get what you pay for.

Unfortunately everyone goes on line looks at a steam boiler price and figures that there contractor is ripping them off and are usually just price or sucker shopping .No matter what math or mark up method one uses as always told to me by a respected plumber was every job is like rolling craps its lucky seven or snake eyes . The other Biggy is are ya getting a complete job ? Any fool can toss in a steamer and roll away but the truth be told as many may know there's a lot more then just replacing the boiler especially when dealing w old systems . Yeah guys prices are high but if ya include doing the rest of the work to put it back to the way it should work then your replacing radiator vents and either adding or replacing main vents . If ya dont want to return for free you will be wanding ,flushing and cleaning the boiler then firing her up and ensuring that all works that all rads are toasty and that the customer is satisfied . usually to make any sort of pay or profit in this day and age the pricing is usually a min of 3 x the cost of equipment and material and maybe you can pay yourself after all is said and done . It seems that most would rather pay less up front and complain either about the price the poor quality of workmanship the over the top fuel costs or the issues of upkeep on older systems which have gotten zero maintenance in 50 to 100 years . The greatest part is being asked on how long some ones mis piped mis sized 40 year old boiler that has seen zero service will last that by far is the best question ever and what can i do to improve on the situation its really a absurd question being i cannot see into the future . Even funnier is do any of these people go to the doctors and ask how long do i have to live and he say i don't know and they accept it but every body thinks where full of pooh and want it all done one shot mean while there milked into monthly dr visits and we feel like a door to door salesmen when we have to call and ask if they want there boiler serviced like money grubbers way to outta hand ,i stopped calling being they'll call when there's a bigger issue and after 3 or 4 years of no service im at a point that even if i did i the install im not interested being they have displayed a lack of trust and instilled a attuide yearly service is not necessary and that its just a money scam ,good for them you have found us out .lol
The biggest misconception about steam boiler replacement is any body can do it and it should not cost much this is why any company financing will always get the job no matter how wrong the piping is its all about the lowest price who cares weather its skimmed or that there's any main vents or 30 year old rad vents they just want the boiler .
Like written by dan i believe years ago i charge to even look at a steam replacement job and it has to be in my area not 1 hour away and it will cost so at least i am getting paid for another waste of my time by someone just price shopping for a low balling rube . i will say as soon as i state i charge for a estimate that usually ends 9 outta 10 calls which is usually fine by me ,tire kickers . But to be fair some times if there real close i dont charge and i go just for the s*its and giggles knowing right off the bat that unless doing for free i will never get the job but as i get older it s become less and less. It also seems the bigger the home the less they will spend so i tell the truth revealing all i see and never holding back ,tell them the truth and some times suggest to ripper out put in hot water or hot air some thing that requires zero home owner inter action and will not even entrain doing that job being there gonna spend 200 k on the kitchen and except to spend 20 g on the heat but bought the home for 2 mil completely brain dead .

I've had a few customers say wow that much? I can buy the boiler online for cheaper. Yes you can! However do you have the knowledge, skills and ability to complete the installation? It all comes at a cost. True, I might be more expensive than another contractor. I choose to educate the customer so they can see the value in what our services provide.
3x the boiler cost is a fair starting point, but have some contingency money as well for like asbestos removal, chimney liners, radiator valves and condensate return lines, etc.

@LRCCBJ NYC require CSD1 safety shutdown on residential installs? Only for commercial boilers in Michigan. I usually install a CO detector for good measure. Good points on the other items depending on what area. Luckily in the Detroit area the we aren't dealing with the added expense of oil conversions. Most of our stuff was coal well and that has been long gone.
