Had a boiler replacement last year, customer had a wet return leak that they kept ignoring plus no maintenance on the boiler for quite a few years and for a few days they forgot to check the water in the boiler. Well the LWCO failed to work because the probe was soo dirty, the boiler continued to fire until all the water was gone, they smelled what they described as fire called the FD they called PSEG and red tagged it. As had been mentioned the loop worked for a little while but poor maintenance is the blame here.
Because it was a Weil-McLain EG boiler one of the gaskets gave instead of cracking the boiler casting. Just to try an experiment I got a new set of gaskets put it back together and hydrostatic tested it and it passed with no leaks.
