A simple question, but the answer I think might back “it depends.”
How do I know when it is time to change the main air vent? Are there any signs or clues I should look for that indicate it needs replacement?

Mostly if it does not close with the presence of steam. Also I suppose it if becomes restricted due to corrosion, etc.

yep it has to vent air and not steam. But I’ve seen them failed closed that you really couldn’t tell from in the living area of the house
