    Why was there no trench box used for the excavation?

    There are rolling trench boxes on pneumatic tires that protect the person in the trench from a cave in of the trench walls.

    NOT large enough to create a sloped escape path. The trench depth rule is still 48" deep without a trench box or shoring.

    Small rolling trench boxes that well drillers use have frames that adjust for the trench width and use 4 pneumatic tires and aluminum plates attached to the frame protect the person in the trench. If OSHA saw that trench there would be a fine imposed.
