How does a radiator vent just fall off?

in Strictly Steam

Friend's house. This is the 2nd one I've seen. Vent just laying of the floor next to the radiator. How does this happen?

    Sloppy threads. May need to drill it for a 1/4" pipe tap and retap and use a 1/4 x 1/8" bushing

    also may have been bumped hard numerous times and damaged the threads.

    Try re-tapping 1/8" pipe thread first. Many times the threads just need cleaning out. If that doesn't work, tap 1/4" pipe thread and install a bushing as @EBEBRATT-Ed said.

    They sometimes rot out at the threads. Do what is said above for the fix.
