    unfortunately these types of accidents often will have reporting inaccuracies, the source is often ill informed, and the journalist is not an expert in the field either. For the curious the best you can hope for is that a CSB investigation is warranted. they publish very in depth reports and often will publish the report as an easily digestible youtube video.

    CO2 tanks can run as high as 2,500#’s. could be something is simple as a fitting valve breaking

    I think @EdTheHeaterMan was referring to the idea that Coca Cola once had a miniscule amount of cocaine in the product. According to my long deceased grandfather, it had a "kick" to it. I suppose today they use caffeine instead.

    CO₃ Molecule is very unstable and quickly reacts to form other compounds.
