Looks like certain HTP boilers use the same HX

Carbon monoxide poisoning from incorrect burner door studs? It’s usually the gasket that’s the problem.
Maybe the studs break off because they were over-torqued at the factory.8.33 lbs./gal. x 60 min./hr. x 20°ΔT = 10,000 BTU's/hour
Two btu per sq ft for degree difference for a slab0 -
Or they were out of dimensional tolerances or they were the wrong material or they weren't heat treated properly.

At least they found the problem fairly quickly its only 800 boilers.
