break out your meter but sounds like a bad end switch

Either that or they just installed a power-stealing thermostat. I've seen this before. The small "stealing" current can make the damper malfunction. An isolation relay will fix this.

It's not necessarily in the damper that the malfunction can occur. Look at the Boiler control board connection to the damper and other connections, too. You really don't need a damper for the boiler to function. As I vaguely recall the damper defaults to open when a failure occurs. If you unplug the damper from the boiler control board, the damper should open, I think. You may have to jump the board connections to get the boiler to fire.

"The damper keeps rotating to the close position and lock off the boiler." What is the sequence that causes this? Is the damper ever in the open position? Is the synchronous motor hot?

Get your meter out. You could have a micro switch or relay problem.

you could lock the damper open for heat, see instructions on the circuit board. It would not surprise me if one of the micro-switches has failed, so the circuit can't tell when the damper is in the open position.

Thanks everyone for your input and support. I lock the damper in open position as 109 A point out. It's working so far while I am waiting on a replacement.

    Much appreciated 👏

Your choice of course but those dampers don't save enough $$$ to pay for the damper. The savings are minute.
