Steam Boiler Rotting Away

Hello I have a steam boiler that was installed in 1998. While servicing it today I noticed the back wall of the block appears to be disintegrating. (SEE PICTURES). It is a natural gas Peerless steam boiler. I don’t see any type of unusual flame shape hitting that area back there. Any thoughts on what may have caused this?

Those are good boilers And it’s only 20 years old.
Dry fired? Or really bad water quality and lots of makeup water.0 -
