DHW Overrun

Hello, I have a Vitodens 200 WB2 6-24 and my DHW continues to overrun my serpoint of 122 degrees. I’ve seen it as high as 158 degrees and had to manually close the feed valve to the tank. I’ve replaced the LGM29 and VR20 as well as the thermo probe on the DHW tank. Thoughts on why it won’t shut off automatically when it’s at temp? Thank you in advance!

    is this a new issue, or continuing ?
    anything else get changed, or fixed sinse issue came on ?

    post a picture showing the boiler and the tank, and the piping and circ in between,
    all in one shot if you can,
