Condensate trap falling off

We have a client in North Jersey and on a Triangle Tube Prestige 175, the condensate trap is falling off. We’ve tightened it multiple times and it seems secure, only to get a call that it’s disconnected. Any ideas? Can pressure somehow build up and force it to pop off?
