That's just a bit vague... can you be more specific?

Is the mixing valve new because the old one wasn't working, and now the new one isn't working?
    Are all 3 new?
High temp, low temp, no temp?
    Circulator is running?
    All circuits are purged, well?
    Toilet flushes?

System is brand new.4zone radiant on a Navien.. 110 heat only boiler. My question is can the mixing valve be defective out of the box? How can I determine and/or test?

Of course it can be bad out of the box, but not likely. I had a bad out of the box thermocouple the other day.
Have you tried swapping mixing valves with one you know works?
    What make and model valve?
What make and model radiant block?
    How many loops?
    What size?
    How long is each loop?
    What make and model circulator?
What's happening (or not happening) that makes the mixing valve bad?

general wide angle view of the bad valve as it's piped and installed,
