Wiring boiler valve thermostat

Cannot figure this wiring to properly wire this to a thermostat so I don't mess it up... does anyone know the proper configuration?

The easiest way is to buy a zone controller like the Taco ZVC.
Just wire it up per instructions.

There are a few ways to tie it all together using a 24 volt transformer but the results are less than professional.

If you are just adding a t-stat to someone else's mess, post a picture and a sketch and we can probably help you out.

The RED wires connect to the end switch (micro switch) and the TT connections on the relay or aquastat. The RED wires turn the boiler on.

The YELLOW wires connect in this order: 24V transformer to thermostat to YELLOW wire thru (the motor) in valve (that closes the end switch) out of the valve to the transformer.
