Burnham freedom error code 12

I have a Burnham freedom gas boiler. This was the boiler that the gas company was giving away and after a few months of having to reset the error code 12 it will not reset anymore. I have trouble shot the boiler. The water level is fine I have 24 volts to transformer anyone here run into this? Thank You.

some people have had problems with bad neutral connections and boilers. your experience seems like a faulty connection steadily getting worse.--nbc

I wonder if it would reset if the water was circulated through the system. Is the LWCO installed in an area that could trap air around the probe? I don't know specifics for that boiler, just tossing out ideas.
