FAN IN CAN WITH ADA-1 Automatic Damper Adapter; WON'T OPEN DAMPER

ADA-1 worked well with fan in the can until….it didn't. Has power, wired correctly, worked for a year and then triggers the fan on, but not open the damper. Anyone have an ADA-1 that died prematurely? Bad relay? Corrosion? Everything checks out but when the fan and ADA-1 are taken out, boiler and damper work fine. So problem is isolated to the ADA-1. ****?

Seems the ADA-1 is just a means to interconnect the three devices. No relays inside of the ADA-1.
I would suspect the fan in a can's safety switch (End Switch). I would use a multimeter and the fan in a can's documentation to prove or disprove my suspicion.
