Actuator Help

Hello there. I'm struggling with replacing an actuator. While replacing it, I'm now trying to figure out if the valve is a mixing valve or a diverter valve. Attached are pics. Can anyone help me determine if it's a mixing valve or a diverter valve, and recommend a replacement actuator? Thank you so much.

It is a 4 way mix valve for low temperature heaters. Do you have radiant heat in the home?
InFloor private labeled that valve, I believe it is a tekmar brand, # 722 so you have other actuator options.
Check both the actuator and that tekmar control to be sure which has a problem.

Yes, I have radiant floor heating, and the actuator hasn't moved in a year. How do I test to see if the actuator has the problem, or if it's the control? here's a closeup of the control

I should add that the valve moves freely when I put the actuator into manual model.

you need a meter to first check power to the tekmar control, then output from the control
Tekmer website probably has a manual with troubleshooting procedures on the control
That control is 25 years old, probably the actuator also
