Anyone ever see a new boiler installed with the burner tubes upside down?

My brand, spanking new boiler (Weil McLain Eg-65) made a huge bang and shut off after its first 30-40 minutes. All the burner tubes were knocked to the ground and the concrete directly underneath them was destroyed (in an area about the size of a large dinner plate, the top layer of concrete had completely shattered so it is like a shallow depression in the floor--and the bits had flown all over, even outside the boiler).

I am trying to figure out what might have made the boiler so darn hot that it made the concrete explode.

One of the members here suggested that the burners might have been installed upside down.

Anyone ever seen that????

Also--should I fear the boiler was damaged by this?

    Replied in other post. I can't see how that would be physically possible, but I can see them maybe not getting hooked on the orfice, and falling down. Rough shipping?

    I need to write an update on the other post, but I am exhausted. It may have to wait until tomorrow.

    No--I looked at them myself before it was turned on. They looked like they were suspended properly (although I did not take a super close look to see if they were upside down!), and then after the super loud "bang", they were on the ground in a pile of rubble.
