    I had a RBI rep provide one of there standard forms. It has most of the pertanent information that you need to be aware of and to do diligence to cover yourself.

    I am sure other boiler manufacturer's have one ready for this request too. I would contact the boiler manufacturer's that you are currently working with to see what is available.
