That looks like something out of <a href="http://fwp.mt.gov/lands/site_281895.aspx"target="_blank">Lewis and Clark Caverns</a>.

This was sitting on the "shelf of shame" over in the boiler code area of my building (dept of labor and industry) and was just one of several things that you don't want to see while looking at a boiler. It was scooped up by one of the boiler/high pressure piping inspectors at some time in the past and I never did get a definitive answer as to the cause of its demise. I first posted this about a year ago and there was a bit of speculation by those here. All I can say is that at some point the pressure was relieved!
