Air Vents

Hi all.
I am curious if the position of an air vent play a part in heating at all? I have one radiator which when I tried to tighten the air vent, the hole faces the ground (upside down). In this case would it affect the heating procedure? And if so, should I use angled vents on such radiator?
Also, I have an old air vent on the main which is stuck or rather, painted over along with the main pipe.
