System start up checklist

Does anyone have a start-up checklist that would apply to HVAC equipment on start-up that we could have installers fill out when the job is complete. Checklist to include peticular information for different installtions and also basic things like cleaning up upon completion and maybe a space for customer signature. If you would have a sheet made up I would appreciate any help on this issue.
