1)Neutral and ground must be isolated to the point of service. That means to the main panel box. If you are using a subpanel you must bring the ground back seperate. So yes to main panel/ no to subpanel

3)How about a fused pullout at the appliance, gives you overcurrent protection and a disconnect all in one.

For a 30 amp, 240 volt appliance being wired in new, can the safety ground be wired to the neutral bar?

If your 30 amp appliance is 3-wire...cap the neutral and use the ground on the receptacle or in the j-box. Always ground. Older panels are bonded anyway and you can use the neutral bar in those cases.
