Does anyone know what the malfunction was this past winter in Mass that caused those two deaths. It was CO blocked by a snow bank and no power for awhile. Did the safety not work? Or was it jumped out?

Are you talking about
the incident in Cape Cod? If so I do not believe that was an Endurance. There was also an CO incident in Vermont at the University with a Heatmaker. I do not have all the details but in both cases the cause was not related to an fault with the equipment but in one case customer blocking vent with snow and also a depressurization inside the home.
The other was an over gassed unit with a recalled Ultra Vent or Plex Vent plastic vent which let go allowing CO to escape into the dorms.0
