Tjernlund
I have been using the Tjernlund PAI series for my boiler installs using a chimney, and for installing on existing jobs that have tight mech rooms. They are easy to install, can stay up near the ceiling, and have been extremely reliable so far....

Fields has the "Fan-in-a Can" combustion air make-up system(you have to supply the connecting pipe).I don't recall the part number, but any supply house should have it available. Tjernlund makes the "In-Forcer" combustion air intake unit. Both interlock with the burner.
Hope this helps.
