Here's a small shop idea

for the on call problems. My sister is a nurse and suggested we do like the independant doctors offices. Share the task with another company. Maybe, just maybe you have a friendly competitor and you could switch-up the on-call schedule. There are logistics to work through, but why not. We have worked with another company in the area, and it works out very well. When you need a much needed break, or have a personal situation that forces you to leave your area, it's nice to have your customers covered.

We bill the other company directly, and they then bill their customer. If doc's can do it, so can you.
