The connection point of the hartford loop into the equalizer is commonly 2" below the normal water line of the boiler. I'm not sure I understand the second part of your question. But if I get it right, it doesn't matter how you get the steam from the boiler to the main as long as you don't reduce the boiler size of the pipe. Always pipe the riser full size from the boiler. By 90* up and over, do you mean a drop header? Always best this way on larger boilers with multiple risers. It makes piping easier and give drier steam, in general.
