When installing an outdoor reset on oil fired boilers that are not supplying low temp loads such as radiant, is a boiler protection loop required to prevent condensation?
If one is required how do you guys usually install them?

If
more than 25% of the load is low temperature, yes install a protection loop or device. If you pipe primary secondary most boiler reset or mixing controls have a protection feature built in, usually a sensor that monitors the boiler return temperature at the boiler return connection.
This control may simply shut down a load till the boiler catches up, or vary the speed of a mixing pump to the radiant.
Some contractors prefer the simplicity of a 3 way thermostatic valve at the boiler. Weil McLain used this method for years on the GV Gold series boilers. Weil and Burnham, among others, have boilers with built in variable speed injection mixing protection. The Burnham Revolution or the Weil GV Gold are two such boilers that do the work for you.
