to tap the side of the steam main, then come out with a short nipple then an elbow up?

I frequently do this. When you tap the hole, hold the tap at a slight upward angle. This will pitch the horizontal nipple slightly back toward the steam main.

Best place to drill and tap is in the side of the last tee supplying a runout.
