Can a boiler be move from the cellar up five stories that will supply the heat for a radiant floor system on four of the those stories without changing the design of a system that was designed start in the cellar and go up, but will now start on the top floor and go down?

    You will have to install...

a low water cutoff in the boiler heat exchanger or close piping (above boiler). If you run the boiler at 20 psi, can the tubing/fittings at the lowest level handle the water pressure (over 40 psi)?

    Expansion tank

You will want to review your expansion tank sizing. It may get smaller.

    It'll Improve...

...air removal, because there will only need to be a few PSI on the boiler. With the circ pump, expansion tank and air separator up near the boiler, any dissolved air will just jump out. Also, large multi-story office or apartment buildings typically have the boilers in penthouses, because it means the boilers don't get tangled up in high pressure code requirements.
