Leaking Wet Returns

Quick question - if I have buried returns in my basement (pipe buried in sand, covered in concrete) and the sand around the pipes is wet (almost like beach sand near the water), is that a sure-fire sign that my returns are leaking?
I have a wet return on either side of my boiler. The one that I thought was bad was made into a dry return last week, but I'm still having to fill the boiler every day. Last night I made a hole to check on the other return, and the sand is soaking wet (just like the one I replaced).

BTW - I know it seems like an obvious question (water+sand=leak), but I'm trying to rule out things like "oh, pipes sweat naturally on their own so it's not a problem", stuff like that.

One more note - I should add that there is NO other source of water/moisture that would make that sand wet, just teh returns.0
