under a vanity in a bathroom or cabinetry in a kitchen be added to the heat in a room or does the majority of it get trapped under the cabinets?

If it's on an outside wall, yes. Definitely help keep the plumbing from freezing.
On an inside wall, I wouldn't bother. You'll just have hot shampoo.

Yes it will help with pipe freezing, although it shouldn't be below freezing temperatures below your vanity if the outside walls are insulated properly
Cutting a small forced air grill in the toe kick will release some of the "locked up" heat.
