installing a recessed steam radiator

I want to replace a floor standing steam radiator with a recessed one (see http://www.governalerads.com/govray.cfm). A plumber will be installing the rad, but I have to do some carpentry.
How does the wall cavity have to be prepared? It's not quite as deep as it should be (4 inches vs. the 5 1/2 inches needed), so I'll have to build out a bit. Does the outer wall get lined with insulation? How's it normally finished?
Sorry - this seems like a simple thing, but I have no examples to observe.
