Here's a stab at it
Heat exchanger: A device utilized to transfer the temperature or heat from one heat storage medium (fluid, vapor or gas) to another. The two mediums are seperated typically by the wall of a plate or tube. The transfer of heat will always occur from the higher temperature medium to the lower temperature medium.
