either the uv sensors aren't working right or they aren't pointed correctly to see the flame. My understanding is that there is a shelf life to these sensors, so even a new one that has been sitting on a shelf too long can be faulty.
Also on that boiler check that the fiber material inside the firebox hasn't falled down and is disrupting the flame pattern. Weil makes a kit and it is very easy to replace it.

repetetive failure of boiler controls
I have 2 Weil Mclain BG-568-WS Boilers in a local large building, they are equipted with Gordon Piatt Burners supplied by weil Mclain model WRG-3-G-05 , 1,155 mbh. The primary control is a honeywell RM7895C1012 with ultra violet flame amplifier. We are a small Heating and plumbing business and do the PM on these boilers and progressively improving the piping as it is quite old. The boilers were replaced about 10 years ago. We have had a recommended company try to get these to keep running,but they continue to fail, almost nightly! They always lockout on #28 "flame failure" Unfortunately we can't catch the failure,as the two units run almost flawlessly during the day or as soon as the units are reset, the primaries have both been changed, The gas piping is 2" which was installed when the boiler was installed. All the wiring has been checked, and the off cycle dampers are both brand new, and operate correctly.Trying to find out if anyone has had a like experience with these burners, or who I might contact for some suggestions . Thankfully the owners are being very patient,but we would like to resolve the situation. we have also contacted weil Mclain and Gordon Piatt , but to no avail! we need input from someone with real world experience Thanks0 -

you need a company that has an on watch burner moniter this device hooks up to a gas or oil burner and takes readings every 5 seconds it will allow you to find the problemthey are expensive pieces of equipment hopefully you can find someone who has one thier web site is www.onwatchinc.com

Lockout
I had a similar problem a while back (different boiler). we had to do a complete reset up of all components. clean the flueways, and (in this case) replace the signal amplifier. Checking to see if no obstructions in the chamber is first, the recheck that the burner is oprating properly (supply and manifold press, air mix etc. Then clean and test the sensor. That was the proceedure recommended to me by the manufacturer (I can't remember who, but it was a vertical fired steam boiler). Haven't had any trouble in 3 years.

flame failure
I had the same problem happen to me recently with what turned out to be an intermittant fuel line delay valve.0 -
Fault Code 28
on the 7800 Series Controls is "Pilot Flame Failure."
That means that the pilot did not light when it was supposed to, or went out before the end of the PTFI Timing.
The G/P Gas Pilot is ignited with a 6K single pole ignition transformer. Test it. If in doubt, replace it. Also check the ignition electrode for porcelain crack(s) or a burned tip. 10 years is a long time. Replace it. The 214 Pilot uses a hook electrode. The distance from the hook tip to the pilot burner tip should be 1/16". The 216 Pilot uses a square tipped electrode. Center it in the hole. These are the 90% fixes.
Other possible problems.
Too much air on start up. Adjust the secondary low fire shutter.
Too much draft. Set the boiler damper for +.10" WC @ high fire.
Insufficient gas pressure @ pilot. We like 3-3.5" between the regulator & pilot.
Pilot air mixing tube blocked or not connected. Very important item to check. The older burners had an adjustable tab @ the burner housing. Make sure it is not closed.
Burner not adjusted properly. Set it up w/ combustion test instruments.
