Problem report form

in THE MAIN WALL
I am trying to put together a problem report form that I can give to a customer that will explain and highlight any deficiency I find during a routine service call.
I would like to use this report to both bring the problem to the attention of the customer and to cover my butt in case a hazard develops.
I would also like it to identify and give and estimated cost of repair.
I would be willing to pay for such a form, does anyone know of any such report?
