How deep do you guys think is deep enough for a slad to be over radiant tubing in a commercial application. I am concerned that a tube could be punctured by a machine mount.

That's tricky
unless you know exactly where stuff willneed to be bolted. For large equipment I build a pattern and install anchor bolts in the slab pour. Or you could make an accurate map of the areas where equipment will bolt in the future.
Yet another way is to keep the tube a bit deeper in the mounting areas. Generally a drop anchor only needs 2 inches of depth to hold. Depends on what you plan on mounting.
