equipment survey forms

just wondering if anyone has a standard equipment survey form that the tech fills out when he enters the customers home to evaluate thier equipment??

    Ma. Code checklist for completion is a great form for most of the needs . (the number escapes me , but if you can get the Dragon to post one , I think you'll be on your way to the info.)

    The only thing I don't remember seeing on the form was information about the dwelling for purposes of determining delivery factors, for oil . (made my own additions in the "comments" section . )

    We (the old oil co. I worked for) did one of these type sheets for every new customer. The info was then loaded into the computer , so that anyone going to a job knew most of the particulars BEFORE going there . Great detail , and the computer program we used had all the blanks to fill in . Hope this helped . Chris
