How do I wire this Grundfos 8620 circulator?

I got a Grundfos model 8620 3-speed circulator along with a used furnace.
The capacitor is not wired. Where do the leads fasten? Is it polarized?
Also the spec plate says "115v", but inside the cover plate is "230 Volt".
Which is right? What is the large Motor screw for?
Thanks

They have a website, www.grundfos.com I thiink. You should be able to get the info there.0 -
What you have
looks like a 15-42 3 speed. The 8620 is the date code. Built in the 20th week of 1986, I believe. Could it be that old of a pump?

That pump does have the removable access to the impeller shaft, which I used to have to spin many of times on those old Grundfos pumps. Do the newer models have them also HR?
