stand-by boiler in a (2) low pressure steam boiler came off pad

We have a (2) boiler installation (low pressure steam) used for several apartment heating in a building. It is a one pipe system and the boilers were recently replaced.
Today morning, one boiler (which was not on) suddenly started shaking and came off the concrete pad.
What could be the reasons?

Hammering at the boiler.
There could be an issue with the Hartford Loop and how it is piped. Also, if it is oil fired, make sure everything is o.k. with the burner and chamber, flame impingement could be the culpit. Bad circulation in the boiler, can also cause this, make sure it is clean and blown-down regularly. By the way do you have a tankless coil on this boiler?
The system could have dry-fired. This is ordinarily accompanied by an explosion but I have seen a 10 section boiler move several feet and tear piping with it on a small episode of dry firing.
