If a boiler was rated at 75k BTU IBR net in the early 1950's and a new boiler from today rated at 75k BTU IBR net are they the same output or are the old ones underrated?

A BTU is a BTU. So they're the same. What is different is how those BTUs are made, collected, stored and delivered.

The measurement method probably has changed some over the years but it isn't like engine horsepower where some methods had no basis in conditions the engine could ever operate in in the real world.

There is an IBR net and IBR gross output number on newer equipment. This article explains how to use the ratings based on where the boiler is installed.
