Each question has a picture associated with it as it's easier than 1,000 words
First one is the end of the pallet is pretty much how high off the basement floor the boiler has to go to leave room for vents on top (4' poured crawlspace). I really wanted a isolation valve between the tee, but it puts me within 3 or so inches of the floor. If I went directly to a tee I'd then have isolation vales on each side, but then I couldn't purge through the loop while isolating the boiler. I suppose I could use purge valves on each side instead. Which is the lesser of two evils?
Second one is the DHW tank sensor is only 1/4" and the well in the indirect is 3/8, but the sleeve makes up the difference. Should I use some thermal paste and how do I hold the sensor in place? It seems jenky to push it in and use electrical tape or something similar.
Third is for the supply sensor. Where do I put that in the schematic above? I assume I just use a tee with a npt fitting, but how far into the flow should the tip of that sensor be?

For the supply sensor I would put a tee in like a pipe x female thread x pipe tee so it has a threaded connection on one of the "runs" of the tee and not the branch of the tee. You want the sensor in as much as possible. It should go into the pipe between the boiler supply outlet and the tee where it splits to go to the indirect and to the system.
The indirect sensor. Buy some wire ties that come with a hole in them for a screw. Screw one in to the indirect jacket with a short screw and tie the sensor wires to it. Thermal paste is a good idea in the well.
For the supply pipe why not put an elbow coming out of the boiler and put the valve on the horizontal and then the tee.1 -
If you are using the sensor as a system operating temperature sensor it needs to be downstream of the system pump a few inches. It measures the mixed temperature coming out of the hydro sep.
The indirect sensor runs the boiler when it goes into DHW priority, probably at 180 f supply
The tip of the sensor well needs to protrude into the piping. If you drop it in too far it may block the flow and cause some turbulence. The actual sensor is at the very tip of those 1/4” thermistors.
If the sensor is sloppy in the well cut a strip of copper tube and make a wedge to hold it in tightly, and some thermal past. Run the wire up the side of the tank, a small screw clip to hold it in the well, against the tank.

the system supply sensor should be located on the system supply side of the low loss header. it is zip tied to the supply pipe between the low loss header and circulator. covered with insulation
the tank sensor can get zip tied to the well also. fold the sensor wire back over the outside of the well and zip tie in the well grove.
install a 90 then ball valve and tee
