Connected Return Supply on Condensing Boiler

We converted from oil to gas about 2 years ago, and have had a bunch of issues with the new unit. I'm questioning if this is installed properly, and if that might be causing some of the issues. My understanding is that the 'cold' return water should flow in the boiler on one side and 'hot' should come out the other side. However, those two pipes are connected on my setup, which seems wrong to me.

    It is normal to have them connected

    Describe the bunch of issues you are having
    the model of the unit
    and photos stepped back a bit to show all the piping

    It's a Carrier BWCAAG000150.
    It seems every month we have a new error code. E010 is the most recent, which says it's a "flame lockout/ACF electronic fault"

    For the past week every morning I wakeup to the error. I need to reset it every 30 minutes till the water temp is above 150 and the house is warm. Once water is up to temp it seems everything runs fine.

    The original installer was in 3 days last week with no real update. He cleaned everything and tried to connect with a Carrier tech to troubleshoot but I guess they are hard to get a hold of. While he was tinkering he also witnessed two additional temp errors of e095 (ACC monitor error) and e088 (iono error).

    A month ago there was 'a clog' which caused things to stop working. Which was a little strange because annual maintenance was done 3 weeks before. Where they also replaced the Water Pressure Regulator - which was causing an error.

    Last year they replaced the expansion tank and I think the flame sensor.

    And there was one other issue, but I didn't get the details as I was traveling.

    All this is within the first 2 years. Very frustrating dropping $10k and then continually having issues.
