Navien boiler throws e109 EVERY WINTER

Our Navien combi boiler throws the air intake error 109 every winter. Sometimes we can reset it by unplugging; sometimes we can reset by taking out air filter and returning (it is never dirty). We just had a tech out yesterday because we had a gas leak around the unit and he replaced some O ring seals. But now every time we run hot water AND the heat it throws the error. Sometimes just leaving the cover off seems to solve it but that can't be good for the unit, right? We live in the Midwest US and the techs don't really know much about these units.

    And if you don't mind, upload photos of the installation? Particularly the vent piping inside and outside.
