PF140 - not igniting (peerless boiler)

The DHW is calling for heat and circulators on however boiler seems to be stuck on DHW Trial for Ignition.  Water temp at boiler is only 94 degrees.   

When I turn the power off and on it works for about 5 days then it happens again.  3rd time in the past few weeks. 

The boiler doesn’t show any errors or lockouts.  Any thoughts?  I’m thinking the control board needs to be replaced.  Unit is about 10 years old.  Flame sensor and igniter were replaced 2 years ago.

    Never seen one stuck on TFI. It's 3 or 5 attempts (I forget which) before A01 ignition hard lockout which will need a manual reset. 
    Is it firing, then losing flame?
    You can access installer setup and check history to see if the flame signal was in range at past ignition trials.
    What's the swirl plate look like?

    Looks like the flame signal was 0 uA the last 5 attempts.  After I power off then on I see it’s back to 6 uA.  

    Seem strange to me that powering off then on fixes the issue for a few days.  Swirl plate seems fine as well.  

    Thinking about switching out the control board.  Besides reconnecting the wires is there any other settings that need to set

    I was also wondering why it does not go to lockout.  Therefore I’m thinking it must be a control board issue…

    I don't want to say throw a board at it but reading 0 uA without going into an A01 lockout is weird.
