one riser or two riser boiler for this application

getting ready to replace the near boiler piping. Is it ok to have only one riser? Or replace Boiler as well with two risers?

You need to find the manual for that boiler and look at the diagram in it. One is probably ok, reducing it like it currently is probably is not (and I wouldn't recommend it even if the manual says it is ok).
Obviously the order is all wrong in the header.0 -

After you make that expensive repair and you still have the same problem, What will you check next?

It appear that the header is built the wrong way. The steam turns and goes to the right of the boiler riser and the equalizer goes to the left of the boiler riser. Any condensate in the header can not get to the equalizer because the boiler riser is in the way. Here is a suggested piping design that may work better.
