Has anyone had this Federal Boiler Airscoop

I have a customer who I observed myself after one turns boiler. in about 3 minutes i hear popping sounds that the water is flashing to steam and caused the prv to dump water consecutively. Its a new PrV and Expansion tank.. It happens really fast starting from cold start... but not all the time. I rebled all zones and had all zones working fine the last time i worked on it and shut boiler down with boiler switch. The Boiler is a Federal Boiler and It has a square box like air scoop. any ideas what the problem can be. here are some pics

    What happens to the pressure gauge as this is happening? What is the precharge in the expansion tank?

    Ahhh, memories. I haven't seen one of those in a long time. 
    You replaced the extrol and relief valve but left the crusty auto vent?
    The pressure only rises when the burner is on? It's not a tankless coil leak?
    Is there an indirect water heater?
    Is the pressure reducing valve working?
