Help with Deciphering my Boiler Service Tag

Hi All,
This question should be real easy for you guys. Every August/September I have the oil company come out and do the usual filter, nozzle, draft test and vacuum/clean the center section of the boiler (if it's sooted up) - whatever is included in my yearly oil contract. When the tech is done he always write his notes on the service card. I'm curious to what it all means. I was working this August when the tech came to the house and I don't feel like waiting till August of 2022 to find out.
Let me know what you guys come up with. Thanks in advance!
8-3-2021 and 8-14-2020 are the two dates I'm curious about.

C & A might mean clean and adjust. O sm probably means 0 smoke reading, -1, -3 are your over fire and breech draft measurements respectively, the others are combustion numbers. Some of them might even be true. The numbers on the right side might be a number assigned to the techs.
Not the most professional. They should leave a printout from the combustion analyzer, for starters. There was an error rendering this rich post.

Thank you Steve. You confirmed pretty much what I thought. The 400 and 450 is probably the stack temperature and I guess the 10% and 8.5% is the CO amount?Long Island, NY0

CO2 or oxygen. CO that high would be a very poorly burning appliance.0

8/3/21 CA= Clean and adjust Check base ......maybe check base of chimney or clean out the fire box base ....guessing
Os would be 0 smoke 8 1/2 Co2, -1 and -3 are probably draft (should be -.01 and -.03) "R" with a circle I think is replace nozzle and filter
@FredoSP how about a few pictures the 8 1/2 if that is Co2 that is low should be over 10 minimum
8-14-21 CA=clean and adjust, NF=nozzle & filter, check base, brush boiler, 0 smoke, -1, -3 is draft (should be -.01 & -.03) 450 sta, 10% ck temp0
